Join a respected not‑for‑profit community care provider supporting older Australians to live safely and independently in their own homes. In this full‑time, 12‑month maximum‑term role, you’ll work with clients across Greater Shepparton and surrounding regions, providing personalised guidance and care planning within a compassionate and values‑driven team.
This position is primarily home‑based, with monthly travel to the organisation’s central office. Accommodation and travel costs are fully covered.

Key Responsibilities
- Engage new clients, review intake information, and establish home care agreements
- Develop personalised, culturally appropriate, person‑centred care plans
- Conduct client visits and provide independent guidance that supports autonomy and wellbeing
- Connect clients with local services, community resources, and partnered providers
- Review and revise annual care plans, goals, services, and budgets
- Maintain compliance with statutory, contractual, and quality standards
- Collaborate with multidisciplinary teams, carers, and external stakeholders
- Ensure ethical, respectful, and confidential service delivery at all times
Key Skills / Qualifications Required
- Previous Care Management experience within Aged Care or community settings
- Diploma or Degree in Health / Allied Health OR 6+ years relevant experience
- Strong communication, interpersonal, advocacy, and relationship‑building skills
- Ability to manage budgets, prioritise workloads, and work independently
- Understanding of Support at Home models, Aged Care Standards, and local service networks
- Current Australian driver’s licence and reliable vehicle
- Proficiency in Microsoft Office and willingness to learn internal systems
- Ability to obtain Police and Working With Children Checks
